Elyse Weber-Sacks, MSW, LSW, CMC
Elyse has a varied and extensive background in social work and business. She is a licensed social worker in both New York and New Jersey. Her social work experience includes medical social work for a large county hospital center in New York City where she was supervisor of the adult and pediatric rehabilitation social work unit. After a change in career direction led her into the business world, she spent many years in brand management and product development working in the field of prestige fragrance.
A return to her social work roots and her lifelong love of the geriatric population brought her to work in admissions/marketing for a large regional nursing home chain, then to a hospice provider where her role was to educate physicians and families about end-of-life options. She also worked for a nonmedical home care company doing client assessments and community outreach.
Elyse joined Connie Rosenberg & Associates Care Management in 2008 and flourished in the role of care manager. Connie was so impressed by her expertise, her work ethic and her compassion that, with an eye to the future, decided to transition the practice to Elyse in 2014. Elyse is honored and humbled to build upon the legacy of this great company.
Elyse received her bachelor’s degree from The University of Maryland/College Park, cum laude and her MSW from New York University’s Silver School of Social Work. She is the immediate past president of the New Jersey Chapter of ALCA and was the 2016 recipient of the Outstanding Chapter member award. Beginning in January 2020, Elyse was elected to the Board of Directors of the Aging Life Care Association.
Elyse is certified as a Geriatric Care Manager by the National Academy of Certified Care Managers and is an Aging Life Care Professional™.

Lisa Monday MSW, LCSW
Lisa received her bachelor’s degree from the University of Miami and her MSW from Rutgers University’s School of Social Work. She is a licensed clinical social worker. Prior to joining Connie Rosenberg & Associates Care Management in 2004, Lisa had been practicing for more than 15 years in psychiatric and medical social work. She has extensive experience in emergency services, outpatient counseling, social work in a home care setting, and recreation therapy services in a nursing home setting. Additionally, Lisa was program director of a large older adult social group. Currently Lisa’s role at CRCM is primarily to assist with weekend On Call responsibilities.

Jacqueline Ornstein, BSN, CMC
Jacqui received her bachelor’s degree in nursing from the College of New Rochelle and has done graduate study in community health nursing. She has a certificate in holistic nursing. Prior to joining Connie Rosenberg & Associates Care Management in 2007, Jacqui had been both a nurse care manager and a program director in an adult day care center in New York, positions she held for nine years. Additionally, she worked as a geriatric care manager in New York for six years. She is certified as a geriatric care manager by the National Academy of Certified Care Managers.

Risa Agin-Levin MSW, LCSW, C-ASWCM
Risa has a bachelor’s degree from The University of Pennsylvania and an MSW from the Wurzweiler School of Social Work at Yeshiva University. She is a licensed clinical social worker. Prior to joining Connie Rosenberg & Associates Care Management in 2014, Risa worked in geriatrics and medical social work for more than 15 years. She has provided geriatric care management throughout northern New Jersey. Additionally, Risa has extensive experience providing care management to residents of assisted living communities and she directed a state-funded in-home assessment program for the elderly and disabled. Her experience includes serving as a social worker for hospital-based home health agencies and with a family service agency in an “Elders at Risk” program. Risa is certified as a geriatric care manager by the National Association of Social Workers.

Laura Broemmer MSW, LCSW, CMC
Associate/Senior Care Manager
Laura received her bachelor’s degree in social work from Florida State University, and a master’s degree in social work from Florida International University. She is a licensed clinical social worker. Prior to joining Connie Rosenberg & Associates Care Management in 2015, her experience over the past 25 years has been in the area of geriatrics, working in skilled nursing facilities and assisted living communities, some of those specializing in Alzheimer’s/dementia care. She has previously worked as a geriatric care manager. She also owned her own consulting company in which she provided consulting and staffing services to skilled nursing facilities. She is a Certified Dementia Practitioner by the National Council of Certified Dementia Practitioners. She is certified as a geriatric care manager by the National Academy of Certified Care Managers.

Pamela has her bachelor’s and master’s degrees in communications from William Paterson University and her associate degree in science from County College of Morris. She is currently completing her bachelor of science degree in nursing at Ramapo College. In addition to providing medication management as her primary role at CRCM, she continues to work as a certified medical surgical registered nurse at Morristown Medical Center (MMC) as well as her private practice as a massage therapist and healing touch practitioner. Pamela works primarily with the geriatric population at MMC and understands the special requirements they have at their stage of life.

Michelle Summer, MSW
Associate/Director of Care Management
Michelle has a bachelor’s degree in sociology and a master’s degree in social work from Kean University. Prior to joining Connie Rosenberg & Associates Care Management in 2020, Michelle worked in the area of geriatrics for 10 years. She spearheaded and ran two grant programs, providing in-home visits to promote aging in place while focusing on preventing unnecessary hospitalizations. She was also a social work manager working in a skilled nursing setting and served on a palliative care team. She was a planning board member and a screener for the National Memory Screening Program, an initiative of the Alzheimer’s Foundation of America. She is a Certified Dementia Practitioner by the National Council of Certified Dementia Practitioners.

Melissa Divney, MSW, LSW
Melissa received both her bachelor’s degree and Master of Arts degree in psychology from Montclair State University, as well as a master’s degree in social work from New York University. She is a licensed social worker. Prior to joining Connie Rosenberg & Associates Care Management in 2020, Melissa had worked for over 20 years in the social work field, including managing the intake department of a community mental health center and, most recently, serving as a social worker in a large continuing care retirement community (CCRC). Additionally, Melissa has experience working in subacute rehab, long-term care, and psychotherapy. In her work roles, she has provided case management, counseling, crisis intervention, advocacy, and support to a diverse clientele.

Robin Solomon, MBA
Robin has a bachelor’s degree in Computer Science and Mathematics from Binghamton University and an MBA in Management from Fairleigh Dickinson University. Prior to joining Connie Rosenberg & Associates Care Management in 2013, Robin was the office manager for a home healthcare agency in Morris County for many years. Her responsibilities included working with prospective families and interviewing and training caregivers. Before that, Robin was a human resources and data quality manager at AT&T as well as a software quality manager at ITT Avionics. Robin continues to volunteer with her pet therapy dog, Brandy, visiting local assisted living communities as well as participating in senior-related community activities.
